Dr. Nesochi is also a designated civil surgeon fully certified by the United States Department of Homeland Security and United States Citizenship & Immigration Services (USCIS) to provide I-693 Immigration medical examinations in office. Dr. Nesochi looks forward to assisting you and your family in your journey towards US citizenship.
Emotional Support Animal Evaluations are offered via telemedicine so that you may get your pet certified as an Emotional Support Animal, if eligible.

INSURANCE & SELF-PAY INFORMATION

If you are a self-pay patient, then this means you are either uninsured or have an insurance plan that is not in-network. Self-pay patients pay in full at the time of the scheduled visit for services rendered. We are not required to file a claim or submit any documentation to a third party.

***Please note that health insurance does not cover the USCIS I-693 medical examination or the Emotional Support Animal Evaluation. Call the office for further information on the out of pocket cost for these evaluations.
If you have a co-pay, deductible, or out of pocket payment due, then you may make the payment via cash or check. For your convenience, payments may also be made via the Zelle payment system using the username: email@example.com
