Before your appointment,  download, complete, and submit your patient registration form. In addition to the completed form, you should also submit a copy of your insurance card, ID, medication list, and any copies of relevant medical records that you have available.

SELF-PAY PATIENTS:

If you are a self-pay patient, then this means you are either uninsured or have an insurance plan that is not in-network. Self-pay patients pay in full at the time of the scheduled visit for services rendered.  We are not required to file a claim or submit any documentation to a third party.
